Medicare’s PDGM Reimbursement model went into effect on January 1st, 2020- and is negatively impacting the amount of therapies patients are receiving under Home Health Services. We believe the amount of therapy delivered to our patients should be determined by the collaboration between therapist and patient, not a business office.

Two streamlined ways to make a Sac Rehab referral:
- Download a treatment referral form and fax it to Sac Rehab Admissions at: 916-218-6319. Please include the demographic sheet and patient insurance information.
