Sac Rehab Solutions provides a more accessible alternative to in-home Physical, Occupational and Speech therapy than provided by home health. We provide Mobile Therapy Services under Medicare Part B and NOT Home Health Services.

Medicare’s PDGM Reimbursement model went into effect on January 1st, 2020- and is negatively impacting the amount of therapies patients are receiving under Home Health Services. At Sac Rehab, we are therapist owned and operated. We believe the amount of therapy delivered to our patients should be determined by the collaboration between therapist and patient, not a business office.

How to Refer

Two streamlined ways to make a Sac Rehab referral:

  1. Download a treatment referral form and fax it to Sac Rehab Admissions at: 916-218-6319. Please include the demographic sheet and patient insurance information.
  2. Submit through our online portal.
